Fast Foods Evaulations | Educator Evaluation

Standards based education programs foster high levels of achievement for all students and help districts and state programs meet curriculum expectations. Our curriculum evaluations are conducted in order to provide information on strengths, needs and recommendations in a variety of program areas. These evaluations are important to our curriculum development and 4-H curriculum accountability. The evaluations provide our 4-H with support, help and assistance in reviewing, evaluating, and designing curriculum programs. As an educator, your input is extremely valuable for these reasons.

Please take the time to fill out the evaluation for the product you are using and return it to:

The evaluations are a Likert based survey. Read each question carefully and circle your best response. We would appreciate any comments.

Participant evaluations are used to assess student learning capacities for each curriculum.  Please take the time to have your students (learners) fill out our participation evaluation and return it to the same address as your educator evaluation.

Volunteers and educators are encouraged to use participant evaluations for your own learning environments to assess your student's learning.
